Consent to issue a passport for a minor

To obtain a passport or temporary passport for a minor, consent of both parents (mother and father) or a legal warden or court-appointed guardian is required.

When submitting a passport application for a minor at the consulate, both parents need to give consent to issue such document there and then in the presence of a consular officer, using an electronic device recording and confirming their handwritten signatures

In case only one parent is present when submitting passport application for a minor, the other parent needs to:

  1. Send via post a written consent consent to issue a passport for a minor (form available for a download below) with his/her signature verified by a Public Notary. When such document is drawn up in Australia, an Apostille stamp is also required.
     
  2. Send to a consular post electronic inbox (ESP):

    Consulate General of the Republic of Poland in Sydney – /KGSydney/SkrytkaESP
    Embassy of Poland in Canberra – /AMBCanberra/SkrytkaESP)

    an electronic written consent to issue a passport for a minor bearing an electronic signature; qualified, trusted or personal.

     
  3. Register consent in person at any passport authority office in Poland or overseas
     
  4. Give his/her consent using e-service: www.gov.pl; >Services for citizens> Documents and personal information>passport> Give consent to issue a passport 

Exceptions:

  1. Consent of parents is not required when they have been deprived of their parental rights or had their parental rights suspended or limited in regards to giving consent for issuing a passport. Please note an original court document/decision confirming the depravation, suspension or restrictions of parental authority regarding giving consent to issuing a passport, needs to be submitted.
     
  2. In the event of parents disagreement or inability to obtain consent from one of the parents, a family court decision giving consent to issue a passport for a minor needs to be submitted. (The original court decision enables only one passport for a minor to be issued at the time).
     
  3. In case one of parents is deceased- an original death certificate needs to be submitted
     
  4. In case a father is unknown- a current, full version birth certificate needs to be submitted
