Starting from 17 July 2021, any person crossing the state border of the Republic of Poland by plane must fill out an electronic Passenger Locator Form via the ICT system before checking in, and if this is not possible, fill out a paper Passenger Locator Form provided by the cabin crew.

The data controller is the State District Sanitary Inspector competent for the place of residence or stay of the traveller. The Passenger Locator Form is located at:  https://www.gov.pl/web/gov/wypelnij-klp
