FORMS FOR MAINTENANCE MATTERS

The Regulation 4/2009 provides nine standard forms concerning maintenance obligation:

  • ANNEX I - Extract from a decision/court settlement in matters relating to maintenance obligations not subject to proceedings for recognition or a declaration of enforceability     
  • ANNEX II - Extract from a decision/court settlement in matters relating to maintenance obligations subject to proceedings for recognition and a declaration of enforceability     
  • ANNEX III -Extract from an authentic instrument in matters relating to maintenance obligations not subject to proceedings for recognition or a declaration of enforceability     
  • ANNEX IV - Extract from an authentic instrument in matters relating to maintenance obligations subject to proceedings for recognition and a declaration of enforceability     
  • ANNEX V - Request for specific measures           
  • ANNEX VI - Application form with a view to the recognition, declaration of enforceability or enforcement of a decision in matters relating to maintenance obligations           
  • ANNEX VII - Application form to obtain or have modified a decision in matters relating to maintenance obligations     
  • ANNEX VIII - Acknowledgement of receipt of an application      
  • ANNEX IX - Notification of refusal or of decision no longer to process an application

 

Abovementioned forms are fully interactive and allow to generate applications in several languages. They are support to simplify communication between the parties of the proceedings and the competent central authorities.
