Take advantage of rehabilitation financed by the ZUS

You are at risk of becoming unable to work due to a disease or injury? Submit an application for rehabilitation financed by the ZUS. Find out how to do this.

Who can take advantage of this service

Anyone who is covered by insurance (for example, a worker or an entrepreneur), is at risk of becoming unable to work and meets one of the following conditions:

  • receives a sickness benefit or a rehabilitation benefit,

  • receives a temporary disability pension for incapacity for work,

  • does not receive any benefits from the ZUS.

What you need to prepare

  1. an application for rehabilitation – you will obtain it from the doctor responsible for your treatment. The doctor may write it by themselves or use the model application for rehabilitation prepared by the ZUS. You may print it or obtain it at a ZUS branch and give it to the doctor. Prepare your data that the doctor will need to complete the application:

  • full name,

  • Personal Identification Number PESEL,

  • residential address,

  • phone number, 

  • email address,

  1. medical records that you will take with you to an examination by a ZUS certifying doctor:

  • documents confirming the diagnosis and treatment of the condition that makes you unable to work, for example hospital discharge summaries, case histories from clinics, descriptions and results of diagnostic tests: X-rays, ultrasound scans and other – if you have them,

  • documents on other conditions, if you suffer from any,

  1. identity card or passport.

What you need to do

  3. Wait for the ZUS to determine when the ZUS certifying doctor will examine you to establish whether you may take advantage of rehabilitation. See How long you will wait for details.

When to submit the application

At any time.

Where to submit the application

At any ZUS branch. You can speed up the process by selecting the branch having jurisdiction over your place of residence.

You may either take the documents to the ZUS or send them by post.

The doctor may send the application for rehabilitation to the ZUS by electronic means. This will speed up the process. Ask the doctor whether this is possible.

How much you will pay

The service is free of charge. The ZUS covers the costs of rehabilitation, accommodation and food and reimburses you for travel costs – the costs of tickets for the least expensive means of public transport.

How long you will wait

The ZUS will handle your application as soon as possible. You will be informed when the ZUS certifying doctor will examine you. If the doctor decides that you need rehabilitation, you will receive a referral. You will agree the time and place of the rehabilitation with a ZUS official. You will wait approx. 8 weeks for the rehabilitation.

Forms of rehabilitation

You may take advantage of the following forms of rehabilitation:

  • inpatient rehabilitation,

  • outpatient rehabilitation.

When you take advantage of inpatient rehabilitation, you remain in a rehabilitation centre the entire time (outside your place of residence).

When you take advantage of outpatient rehabilitation, you come for planned treatment and medical consultations to a rehabilitation centre every day (you do not stay there the entire time).

You may take advantage of inpatient rehabilitation for:

  • locomotor system disorders – if, for example, you have suffered an injury (a fracture, a sprain) or you suffer from back pain, sciatica, osteoarthrosis or arthritis,

  • cardiovascular disorders – if, for example, you suffer from ischaemic heart disease or arterial hypertension, you have had a heart attack or you have undergone angioplasty procedures or a cardiac surgery,

  • respiratory disorders – if, for example, you suffer from bronchial asthma or chronic obstructive pulmonary disease (COPD),  

  • psychosomatic disorders – if, for example, you suffer from mood disorders, neurotic disorders or related somatic disorders,

  • oncological disorders – if you have undergone breast cancer treatment,

  • voice disorders – if, for example, you suffer from hoarseness, chronic laryngitis or periodic loss of voice, especially if your voice is your livelihood.

You may take advantage of outpatient rehabilitation for:

  • locomotor system disorders,

  • cardiovascular disorders.

Duration of rehabilitation

24 days, but, depending on your situation, the rehabilitation may be:

  • extended – if, for example, the doctor decides that this will restore your ability to work,

  • shortened – if, for example, your ability to work is restored earlier.
