Where can I apply?
If you lose a passport or temporary passport abroad, notify the loss immediately to a Republic of Poland consular office.
Filing a passport loss notice with the consulate means that your document loses its validity as of the date of notifying the consul about passport loss.
Do I need to file documents in person?
In order to notify the loss of a passport or temporary passport, go to the nearest or selected consular office.
You do not need to notify passport loss personally in the consulate if you have a trusted profile, qualified certificate or e-identity card. In such cases, you can notify the loss or damage of your passport document either in writing using an electronic form or online via the provided electronic service.
In addition, you can also notify the loss of a passport document by means of a written letter with a signature certified as your own by a notary.
Remember! If you find the passport whose loss you have notified to the consulate, you should immediately return the document to the passport authority (the consul if you are abroad).
How do I make an appointment?
If you want to notify the loss of a passport at the consulate, you do not need to make an appointment.
What documents do I need to submit?
The consul will receive your passport or temporary passport loss notice in electronic form during a passport visit in the consular office. A consular employee will file your application using an electronic form and you need to confirm that the data are correct by placing your signature on a dedicated device (signature pad).
Your passport document is no longer valid as of the date of filing the notice.
Remember! If you find your passport document whose loss you notified to the consul, you cannot use such document or withdraw the loss notice. A found passport or temporary passport should be returned to the passport authority having territorial jurisdiction (regional governor in Poland, consul abroad). You cannot receive the passport back.
You are obliged to return a passport document regardless of whether you already have another document issued to replace it or you did not apply for a new document.
Any attempts to identify yourself using such document when crossing the border may result in serious consequences and hindrances.

Filing a notice is free of charge.
How long do I have to wait?
The consul will accept your document notice loss immediately.

What should I do if my passport document is lost or destroyed and I need to urgently return to my place of residence?
If you lose your passport document abroad, the consul will, on your request and after confirming your identity and citizenship, issue to you a temporary passport allowing you to return to your place of residence (in Poland or elsewhere).
Remember that an honorary consul cannot issue you a temporary passport.
