C-Type Schengen Visa
Where to apply?
You can apply at the consular office, Street 24, G-5/4, Diplomatic Enclave II, Islamabad.
After a prior registration through e-konsulat, (no walk-in).
Do I have to apply in person?
Visa application form must be submitted in person.
Diplomatic mission does not accept application forms sent via fax, regular mail or e-mail.
How to book an appointment?
You can fill out the visa application form via the e-konsulat system.
Please be informed, that the e-Consulate system www.e-konsulat.gov.pl , owned by Ministry of Foreign Affairs of the Republic of Poland, is the only official online registration system for Polish consulates worldwide.
Please do not use any other websites, which are claiming to offer visa appointments for the Embassy of Poland in Islamabad.
All persons planning to apply for visa are strongly advised to be extremely cautious in all dealings with companies that claim to offer assistance in obtaining visa, and that charge money for preparing documents for visa procedure.
What documents do I need to submit?
1. A visa application form filled via the e-konsulat system, printed and signed.
2. A 3.5 x 4.5 cm colour photo. The photo must be:
a. sharp, taken against white background and printed on a quality paper,
b. newer than 6 months,
c. taken en face, clearly showing the eyes and face from both sides from the top of the head to the top of the shoulders with the face covering 70-80% of the photo. The photo must be taken without any headwear.
3. Passport issued within last ten years, valid for at least three months from the expected return date, with at least two blank pages for visas, as well as all previous passports and if you have another valid passport, you should attach it to the visa application form.
4. A copy of the passport page with personal data and a photo.
5. Healthcare insurance valid across all countries of the Schengen area for the amount not lower than EUR 30,000. If you apply for a multiple-entry visa, it should cover the date of the first entry.
6. Copy of an ID document to confirm residence within the consular district of your application.
7. Proof that you have enough means of subsistence in the form decided by the mission:
a. Short-stay visa application form duly filled and signed by the applicant. In case of minor(s), form should be signed by both parents.
b. Two (2) recent photographs of the applicant.
c. An ordinary passport or other recognized travel document valid for at least three months from the expiry date of the visa applied for. This document must have at least two empty pages. Please bring all your previous passports, if any, and attach copies of used pages with your application (on A4 size paper).
d. Photocopy of your national ID card.
e. Third country nationals residing in Pakistan must provide supporting documents proving legal residence.
f. If you have been ever refused a visa by any Embassy or High Commission a refusal letter will be required.
g. Original and photocopy of Family Registration Certificate issued by NADRA (in English or with a certified translation if in Urdu).
h. For underage children (under 18) travelling with a single parent, a written consent of the other parent is required with photocopy of his CNIC card (personal presence may be required).
i. Original signed letter of the applicant’s company/employer stating name, position, salary, duration of employment, address and contact numbers of the employer, purpose and duration of visit to the Schengen state(s). If the applicant is a “student”, original signed letter from the applicant’s school/college or University.
j. Original bank statements of the last six months, signed and stamped by the bank branch manager. Last three salary slips, (if salaried),
k. Accommodation proof (confirmed hotel reservation, rent agreement clearly stating your name, rental period dates and accommodation address) covering the entire period of travel.
l. Copy of flight booking(s) – both ways.
m. If the trip is a package tour, attach detailed itinerary.
n. Original AND copy of a travel medical insurance policy of not less than 30,000 Euros. The policy must cover medical and hospitalization expenses for the entire period of valid stay on Schengen territory as well as repatriation costs. Consult valid list of authorized insurance companies in Pakistan as agreed by Schengen partners in Islamabad
9. A document to confirm the purpose and the conditions of your planned stay in Poland [please find the list below].
10. Family Registration Certificate issued by NADRA.
Business Purpose
1.Short-stay visa application form duly filled and signed by the applicant.
2.One recent photograph of the applicant (passport size, on white background only, taken not earlier than 6 months before applying for visa).
3.An ordinary passport or other recognized travel document valid for at least three months from the expiry date of the visa applied for. This document must have at least two empty pages. Please bring previous passports, if any, and attach with application copies, on A4 size paper, of used pages.
4.Photocopy of your national ID card.
5.If you have been ever refused a visa by any Embassy or High Commission a refusal letter will be required.
6.Third country nationals residing in Pakistan must provide supporting documents proving legal residence.
7.Original signed letter of the applicant’s company/employer stating name, position, salary, duration of employment, address and contact numbers of the employer, purpose and duration of visit to the Schengen state(s).
8.Original personal bank statements of the last six months. National Tax Number Certificate (NTM) for the previous two years and proof of income tax payment (FBR).
9.Documents of the organization’s income: Original organization’s bank statements of the last six months, National Tax Number Certificate (NTN) for the previous two years and proof of income tax payment (Federal Board of Revenue), Chamber of Commerce Certificate valid for the current year, Recommendation for business visa from the Chamber of Commerce.
10.A business invitation sent by Polish company, including personal details of the invited person (position in the company), purpose of visit, time and duration of stay in Poland and all contact details in Poland. One copy of the invitation has to be sent directly to the consular/visa section of the Embassy of Poland in Islamabad (by fax or e-mail).
11.Covering letter - should contain short information about the company, about the transaction with the Polish company, purpose and time of the visit to Poland.
12.Proof of business activities with the company in Poland.
13.Photocopy of the latest correspondence with the Polish company.
14.Confirmed hotel reservation in Poland.
15.Copy of flight booking(s) – both ways.
16.Original AND copy of a travel medical insurance policy of not less than 30,000 Euros. The policy must cover medical and hospitalization expenses for the entire period of valid stay on Schengen territory as well as repatriation costs. Consult valid list of authorized insurance companies in Pakistan as agreed by Schengen partners in Islamabad.
The Embassy reserves its right to request additional documents.
Original documents not needed will be returned with passport but only if copies are provided by applicant as requested.
Health purpose - Medical treatment
1.Visa application form duly filled and signed by the applicant. In case of minor(s), form should be signed by both parents.
2.One recent photograph of the applicant (passport size, on white background only, taken not earlier than 6 months before applying for visa).
3.An ordinary passport or other recognized travel document valid for at least three months from the expiry date of the visa applied for. This document must have at least two empty pages. Please bring previous passports, if any, and attach with application copies, on A4 size paper, of used pages.
4.Photocopy of your national ID card.
5.Third country nationals residing in Pakistan must provide supporting documents proving legal residence.
6.If you have been ever refused a visa by an Embassy or High Commission the refusal letter will be required.

10. Supporting documents of personal income : Original bank statements of the last six months. Last three salary slips
11. Medical certificate from a medical doctor or medical institution stating the need for a specific treatment to be received in an establishment located in the territory of the Member States; and/or
- an attestation stating the need for the treatment to be delivered by the physician accredited with/or designated by the Member State consulate and whose consultation fees will be paid by the applicant;
12. Letter on official letterhead from the receiving medical establishment confirming that the specific treatment can be performed on the patient who had given his/her consent accordingly;
13. Proof of sufficient financial means to cover the expected comprehensive treatment and proof of pre-payment of the expected comprehensive treatment if applicable;
14. Correspondence between the sending and receiving medical practitioners or establishment describing the arrangements made for the specific medical treatment including expected arrival of the patient and expected duration of the treatment, as well as describing the name of the patient's attendant(s) if applicable as well as facilities extended to them during the stay of the patient;
15. In case of transplants, certificate of registration on the national list of patients awaiting transplants, issued by the surgical team who intends to undertake the operation.
16. Original AND copy of document (NADRA Family Registration) showing family relationships between the applicant and the sponsor (in English or with a certified translation if in Urdu).
17.Copy of flight booking(s) – both ways.
18.Proof of accomodation in Poland.
19.Original AND copy of a travel medical insurance policy of not less than 30,000 Euros. The policy must cover medical and hospitalization expenses for the entire period of valid stay on Schengen territory as well as repatriation costs. Consult valid list of authorized insurance companies in Pakistan as agreed by Schengen partners in Islamabad.
20.The Consulate might ask for additional documents if found necessary.
The Embassy reserves its right to request additional documents.
Original documents not needed will be returned with passport but only if copies are provided by applicant as requested.
Private Purpose / Family Visit
REQUIREMENTS:
1.Visa application form duly filled and signed by the applicant. In case of minor(s), form should be signed by both parents.
2.One recent photograph of the applicant (passport size, on white background only, taken not earlier than 6 months before applying for visa).
3.An ordinary passport or other recognized travel document valid for at least three months from the expiry date of the visa applied for. This document must have at least two empty pages. Please bring previous passports, if any, and attach with application copies, on A4 size paper, of used pages.
4.Photocopy of your national ID card.
5.Third country nationals residing in Pakistan must provide supporting documents proving legal residence.
6.If you have been ever refused a visa by an Embassy or High Commission the refusal letter will be required.

11.Formal invitation letter called "ZAPROSZENIE" (in original), issued by the local authorities in Poland (Urząd Wojewódzki) on the request of inviting person.
If the person you are visiting in Poland is also responsible for your means of support, then he/she should write a sponsorship letter with full contact details in Poland and attach :
copy of his/her bank statements of the last 3 months;
copy of his/her last three salary slips;
copy of his/her last tax form;
copy of the identity card/passport, if Polish citizen, or Polish resident card.

14.Proof of accomodation in Poland.

16.Copy of the identity card/passport of inviting Polish citizen, or Polish resident card.

Sports competitions
1.Visa application form duly filled and signed by the applicant. In case of minor(s), form should be signed by both parents.
2.One recent photograph of the applicant (passport size, on white background only, taken not earlier than 6 months before applying for visa).
3.An ordinary passport or other recognized travel document valid for at least three months from the expiry date of the visa applied for. This document must have at least two empty pages. Please bring previous passports, if any, and attach with application copies, on A4 size paper, of used pages.
4.Photocopy of your national ID card.
5.Third country nationals residing in Pakistan must provide supporting documents proving legal residence.

A letter on official letterhead of the national federation featuring information on the competitor/athlete, including his/her Pakistani Identity Card (CNIC) number, his/her national ranking, mentioning the sport event where participation is intended, and/or names and position of people accompanying competitor/athlete if applicable;
12. Invitation to participate from organizers located in the territory of Poland and evidence of enrolment in the event and programme;
13. Evidence of insurance covering a sports event, covering potential injuries incurred during training or competition.
14. Original AND copy of document (NADRA Family Registration form B) showing family relationships between the applicant and the sponsor (in English or with a certified translation if in Urdu).
15.Copy of flight booking(s) - BOTH WAYS.
16.Proof of accomodation in Poland.
17.Original AND copy of a travel medical insurance policy of not less than 30,000 Euros. The policy must cover medical and hospitalization expenses for the entire period of valid stay on Schengen territory as well as repatriation costs. Tourist Purpose
REQUIREMENTS:

Additionally, when applying for a visa for a minor you should present:
- Written consent of the child’s parent(s) to apply for a visa, confirmed by a notary. If there is only one guardian, it should be proved by a birth certificate, a court decision on exclusive parental custody or a death certificate of the other parent.
- Originals and copies of parents' passports.
- Original and a copy of a birth certificate.
If a child is entered in their parent's or a legal guardian’s passport, please submit a separate visa application. The visa will be pasted into the parent's or the legal guardian's passport.
Please note:
- You must apply for a visa in a consular office or the embassy’s consular department in the consular district of your place of residence.
- Usually, the above documents are enough to receive a visa, however, the consul may require additional documents.
- Apply for a visa not earlier than 6 months and not later than 3 weeks before your planned journey.
- The consul may, invite the applicant for an interview during the processing.
- You may be banned from entering any of the Schengen States if you present forged documents or provide false information.
- Receiving the visa does not guarantee that you will enter the Schengen area - the final decision is always made by the authorities of the country where you intend to cross the Schengen area border.
How much does it cost?
- Visa application costs 28 035 PKR.
The visa application fee is non-returnable, whatever the consul's decision.
What is the waiting time?
The consul will make the decision on the visa within 15 calendar days.
Exceptionally, the time to make the decision can be extended up to 45 days.
How to collect the documents?
Personally by an applicant or an authorized person (an authorized person should provide his/her an original ID card (CNIC), the authorization letter and visa receipt).
Collections: business days between 1:00PM- 3:00PM.
How to appeal?
Within 14 calendar days starting from the day of collecting the refusal decision.
No prior appointment is required. Appeals can be submitted on business days bewteen 11:00AM-3:00PM.
Appeals must be submitted personally or by authorized person (an authorized person should provide the copy of your passport, his/her an original ID card (CNIC), the authorization letter and required documents).
Required documents: passport – must be valid; appeal – signed by applicant; refusal of visa and other documents which in your opinion might be important.
Appeals sent via email or post will not be considered.
Appeal processing fee is 24 752 PKR.
Processing time is 14 calendar days.
Revocation or annulment of Schengen visa
If you have received a decision to revoke or annul your Schengen visa that you do not agree with, you have the right to ask for a reconsideration. However, if you have applied yourself for your Schengen visa to be revoked and the consul has agreed, you are not entitled to appeal.
You should apply for reconsideration within 14 days of receiving the decision to annul or revoke your Schengen visa at the consular post that issued the decision.
No prior appointment is required. Wednesdays and Thursdays 10.00 AM - 1.00 PM.
If you apply for reconsideration after your Schengen visa was revoked or annulled, you do not have to pay the consular fee.
Complaint about refusal to issue Schengen visa, decision to revoke or annul Schengen visa and the consul's activities in this respect.
If the consul who examines your reconsideration request, issues a new decision to refuse, annul or revoke your visa, which you do not agree with, you have the right to submit a complaint to the Provincial Administrative Court in Warsaw. The complaint should be forwarded through the consul who made the decision, within 30 days of receiving the decision taken as a result of the re-examination.
You do not pay a consular fee for submitting a complaint to the consul, but you have to pay fees for proceedings before administrative courts. Therefore, you have to note that the court will ask you to pay the court fee. More information on the amount of court fees when appealing with the administrative court, possibility to apply for an exemption from the costs and the language of the procedure, is available at:
