Consent to issue a passport document for a child

Consent of the mother and father, a legal caretaker, or a court-appointed guardian is necessary to obtain a passport/temporary passport for their child or a child in their custody.

 

When both parents submit the application at a consulate, the consent to issue a passport for a child is given before a consulate official, using a device which can record a handwritten signature.

 

When only one of the parents applies for a passport, the other can:

 

  • send to the consular post’s electronic inbox (ESP address: Ambasada Rzeczypospolitej Polskiej w Królestwie Tajlandii z siedzibą w Bangkoku, /AMB_Bangkok/SkrytkaESP) the consent to issue a passport document to a child signed with an electronic signaturequalified, personal or trusted signature:
  • deliver to the consulate in person or by mail the original of the written consent to issue a passport document for a child with a signature certified by a passport authority or a notary;
  • register the consent personally before a consul or at a passport authority in Poland.

 

Exceptions:

  • consent is not required from a mother or a father who have been deprived of their parental rights or had their parental rights limited regarding giving consent to issuing a passport document; in this case, you should supply the consul with the original of a court judgement confirming the deprivation, suspension, or restriction of parental authority regarding giving consent to issuing a passport document;
  • if the mother and the father disagree or the consent cannot be obtained from one of the parents, the consent to issue a passport document can be replaced with a family court’s decision, the original of which should be presented when submitting the application for a passport document (based on the court’s decision, only one passport document can be issued);
  • if one of the parents is dead, a death certificate must be presented
  • if the father is unknown, a current full copy of birth certificate must be presented

 

Registration of consent in the Consent Module by a consul

 

Registration of consent to issue a passport document for a child is a free-of-charge consular activity, independent from the application for issuing a passport document for a minor.

Consent can be registered at a consulate without making an appointment during its office hours.

When registering your consent you must show a valid personal ID or a valid passport document.
