Where do I report the loss of my document?
If you are abroad, report a lost document immediately in any Polish consular office.
IMPORTANT! Honorary consuls do not accept reports of lost passports.
Do I need to report the loss in person?
To report a lost passport or temporary passport, visit your nearest consular office or any consular office of your choice.
If you have a verified profile ("Profil Zaufany"), a qualified certificate, or an e-identity card ("e-dowód"), you do not need to notify the consular office in person. In such cases, you can report the loss or damage of your passport in writing, through an electronic form, or online via the available electronic service.
Alternatively, you can report the loss of your passport by submitting a written letter, with your signature certified by a notary public.
If you locate your lost passport after reporting it, you must promptly return it to the relevant passport authority (the consul if you are abroad).
IMPORTANT! Honorary consuls do not accept reports of lost passports.

What documents do I need to submit?
- The consul will accept your report electronically during your visit to the consular office. You will need to confirm that all information is correct and sign it on a designated device (SignaturePad).
- Your passport becomes invalid as of the date you report its loss.
- Please note, if you find your lost passport, it can no longer be used, and you cannot retract your report.
- If found, the passport must be returned to the relevant passport authority (province governor in Poland or consul abroad). You will not be able to reclaim the passport.
- You must return the reported passport, regardless of whether you have already obtained a replacement or have not yet applied for a new one.
- Attempting to use the lost passport at a border crossing could result in serious consequences and complications.

What happens if I lose or damage my passport abroad and need to return to my place of permanent residence?
If you lose your passport abroad, upon your request and after checking your identity and citizenship, the consul can issue a temporary passport for you to return to your place of permanent residence (in Poland or in another country).
IMPORTANT! Honorary consuls cannot issue passports.
