Temporary passport for a minor

Where to apply?

You can apply for your child’s temporary passport in any Polish consular office. Please remember that you will apply for and collect your child's new temporary passport in the same office.

Do I have to apply in person?

Temporary passport application form for a child should be submitted personally by:

  • both parents or court-appointed guardians

or

  • one of the parents or court-appointed guardians,

if they present:

  • written consent of the other parent (guardian) to issue a passport to the child containing the confirmation of the signature's authenticity by a notary, consul or a passport department official authorised by the province governor,
  • final and binding court decision under which the other parent (guardian) has no right to make decisions about issuing a passport to the child,
  • final and binding family court decision in which the court, 
  • consents to the issue of a passport to a child without the other parent's consent,
  • unabridged copy of a Polish birth certificate stating that the father of the child is unknown (there was no recognition of fatherhood or judicial establishment of paternity),
  • copy of a death certificate of the other parent.

A child over 5 must be present when you submit the temporary passport application.

How to book an appointment?

You can book your appointment to apply for a temporary passport via the e-konsulat.

What documents do I need to submit?
  • Legibly completed passport application form which may be collected from the consulate or downloaded from its website. 

Parents give their consent to the issue of a temporary passport on the application form by signing it in the presence of a consular official and present their valid passports or personal IDs.

  • Colour photo - read what the photo should look like.
  • Abridged or unabridged copy of a Polish birth certificate if the child does not have its PESEL number.
  • The child's current passport or temporary passport (for review only), if it had already been issued.

If the application is made by one of the parents, please prepare also one of the following documents:

  • written consent of the other parent or legal guardian containing the confirmation of the signature's authenticity by a notary, consul or a passport department official authorised by the province governor,
  • final and binding court decision under which the other parent (guardian) has no right to make decisions about issuing a passport to the child,
  • final and binding family court decision in which the court consents to the issue of a passport to a child without the other parent's consent,
  • unabridged copy of a Polish birth certificate stating that the father of the child is unknown (there was no recognition of fatherhood or judicial establishment of paternity),
  • copy of a death certificate of the other parent.

The consul may ask you to present your child’s additional documents if he/she cannot establish its identity and citizenship based on the data in the documents presented or the data available to the consul in his/her records and registers, or if the data are unclear or inconsistent:

  • abridged or unabridged copy of a Polish birth certificate,
  • decision of the province governor confirming the child's Polish citizenship,
  • valid foreign identity document.
How much does it cost?

Fee for a temporary passport issued for the delivery time of the biometric passport is 30 €

Fee for a temporary passport issued for other purpose than to cover the biometric passport delivery time is 15 €

The fees must be paid at the office counter when submitting the application.

Accepted forms of payment: cash.

What is the waiting time?

You will receive the child's temporary passport as soon as possible.

Please note, if you are planning to take your child abroad with the temporary passport, check current rules of entry and stay of foreigners in the country of destination. Make sure that your child can cross the border with the temporary passport and check the required validity period (at entry and departure).

How to collect the documents?

The child's temporary passport can be collected by either of the parents (guardians) personally in the same office where the application was submitted.

On the collection day of the new document, bring the child's previous passport (or temporary passport) to the consulate - current passport will be cancelled, except for pages with valid visas, and returned.

Can I get a temporary passport?

A temporary passport is issued only under specific circumstances. It can be obtained by individuals who:

  • stay in Poland and abroad temporarily and want to return to their place of permanent residence,
  • stay abroad and are waiting for the delivery of a passport issued in the Republic of Poland (if they express such wish and apply for such document), 
  • stay in Poland and abroad, in urgent cases of a family member's illness or funeral of which they need to provide evidence,
  • stay in Poland and abroad, in urgent cases related to their professional activities of which they need to provide evidence,
  • cannot have their fingerprints taken physically, and the impediment is temporary.

Can I apply for a temporary passport for a child if I lost its personal ID card and the child's new passport is waiting to be collected in Poland?

You can apply for a temporary passport for a child at a consulate. Remember to take with you the temporary passport issued by the consul as you return to the country to collect the new one.
  
I lost my child's passport. What to do now?

If you lost your child's passport or temporary passport, you must immediately inform the passport authority - the authority abroad is the consul.

On the day when you inform the consulate about the loss of passport, the document becomes invalid. 

Important! If you find the passport which you had reported lost in the consulate, you should immediately return it to the passport authority (to the consul, if you are abroad).
