Reporting a lost passport or temporary passport

Where to apply?

If you are abroad, report a lost passport or temporary passport immediately in a Polish consular office.

Do I have to apply in person?

Go to your nearest consular office or a consular office of your choice to report a lost passport or a temporary passport. 

Honorary consuls do not accept reports of lost passports.

What documents do I need to submit?

The consul will accept your report of lost passport or temporary passport after you fill in a form available at the consulate.

On the day of the report your passport becomes invalid. After making necessary checks, the consul will enter immediately the information on invalidity of your passport or temporary passport into the central passport register.

Please note, if you find the passport you had reported lost, you cannot use it any longer nor withdraw the report. If found, the passport or temporary passport should be returned to the passport authority of local competence (province governor in Poland, consul abroad). You cannot have it back.

You must return the passport whether you have already received the replacement passport or you haven't applied for a replacement passport at all. 

An attempt to use the document while crossing the border may lead to serious consequences and difficulties.

What happens if I lose or damage my passport abroad and need to return to my place of permanent residence?

If you lose your passport abroad, upon your request and after checking your identity and citizenship, the consul can issue a temporary passport for you to return to your place of permanent residence (in Poland or in another country).  

Remember that honorary consuls cannot issue a passport.

What happens if I lose my passport in a country without a Polish consul and I need to return to my place of permanent residence?

If you lose your passport while staying in a country without a Polish consulate, you may ask a consul of another European Union member state to issue an EU Emergency Travel Document (ETD). 

This document can be issued for a single journey back to Poland or to another EU member state, after your country of citizenship has given its consent for it. In the case of Polish citizens, the consent is given by a competent Polish consul.

If you lose your passport, first get in touch by phone or e-mail with the Polish consul in Kuwait, also serving his duty in Bahrain. He/she will tell you what further steps you need to take to get the Emergency Travel Document.

If your lost passport contained a visa on which you had entered the country, check the procedure of leaving. It may be complicated to leave some countries without proper visa and entry stamps.

While planning your return trip, keep in mind that the EU Emergency Travel Document does not authorise you to enter any other countries besides the EU member states.

Please note that an honorary consul cannot issue an Emergency Travel Document (ETD).
