For specialist doctors, telephone registration is available Monday to Thursday from 7:00 a.m. to 7:00 p.m. and on Fridays from 7:00 a.m. to 6:00 p.m.

Appointment Cancellation
You can cancel your appointment at a clinic using the provided email address: rezygnacja.wizyty@cskmswia.gov.pl
Please note that this email address is exclusively for canceling appointments at Specialist Outpatient Clinics and Primary Care Outpatient Clinics (POZ).

Patients unable to attend their pre-scheduled appointment for various reasons are kindly requested to contact the secretariat of the respective outpatient clinic to cancel by phone.
Initial appointment enrollment requires a referral from a health insurance doctor.

Central Registration - Building C (Budynek C)
Monday - Thursday: 7:00 am - 7:00 pm
Friday: 7:00 am - 6:00 pm
- For the Dermatology Outpatient Clinic, enrollment takes place on the 6th floor of Building D at the registration desk of the Dermatology Clinic until 7:00 p.m.
- Enrollment for the Outpatient Clinic of Pancreatic Diseases and the Outpatient Clinic of Bowel Diseases is conducted on the 3rd floor of Building D in the Department of Internal Medicine and Gastroenterology until 2:00 p.m.
- Registration for the Alzheimer's Outpatient Clinic and the Outpatient Clinic for Extrapyramidal Disorders is available on the 1st floor of Building B until 2:00 p.m.

Friday: 7:00 am - 6:00 pm

We encourage you to visit a general practitioner (GP).
A referral is not required for the services of the following specialists:
- Gynaecologist and Obstetrician,
- Oncologist,
- Psychiatrist,
- Venereologist,
- Dentist.
Individuals receiving outpatient specialist care within the following categories are exempt from the referral requirement:
- War invalids,
- Repressed individuals,
- War veterans,
- Blind civilian war victims,
- Tuberculosis patients,
- Individuals who are HIV positive,
- People with severe disabilities,
- Those undergoing organ donor testing,
- Individuals struggling with alcohol, narcotic drugs, and psychotropic substances addiction, specifically for drug treatment,
- Entitled individuals (whether soldiers or employees), and veterans, particularly for the treatment of injuries or diseases acquired during missions beyond the state's borders

What to bring to the hospital?
Dear Patient,
the scheduled date for your hospital admission is determined by the admitting doctor in consultation with you. To ensure a smooth admission process, please make sure to have the following documents with you:
- Referral to the hospital
- Proof of identity
- Necessary test results
In addition, please ensure that you possess valid health insurance, which can be confirmed through the EWUŚ system. If there is no valid insurance information available in the aforementioned system, you have the option to provide a declaration of valid insurance. Children and adolescents up to the age of 18 who hold Polish citizenship (confirmed, for instance, by the identity cards of both parents) are entitled to benefits.
During your hospital stay, it's important to have personal items on hand. Consider packing the following:
- Shower sandals/Slippers,
- Pyjamas or other sleepwear,
- Bathrobe,
- Comfortable cotton clothing, like a tracksuit,
- Underwear,
- Towel,
- Toiletries,
- Cutlery,
- Mug,
- Still mineral water,
- Tissues,
- Books and magazines,
- Mobile phone with charger.
