You can apply for your child’s passport in any Polish consular office. Please remember that you will apply for and collect your child's new passport in the same office.

You can apply for a passport for a child at any consular office of the Republic of Poland. Remember that you will pick up your new passport at the same office where you apply for it.

The application for a passport for a child is submitted in person by:
• mother, father or, respectively, a legal guardian or curator
or
• one of the parents or court-appointed legal guardians or a probation officer if he submits:
1. written consent of the other parent to issue a passport to the child, including confirmation of the authenticity of his/her signature by the passport authority (voivode, consul) or notary public; the parent may consent to issuing a passport to the child also by sending to the Electronic Inbox [please enter the address of the mailbox] of the consular office a letter of consent with a qualified electronic signature, a trusted signature or a personal signature
The consent should include:
1) first name(s), surname, PESEL number, if assigned, and in the absence of a PESEL number - date of birth, as well as the series and number of a valid document confirming the identity of the parent giving consent;
2) first name(s), surname and PESEL number, if assigned, and in the absence of a PESEL number - date of birth of the minor to whom the application for a passport document relates.

2. a final court decision stating that the other parent has no right to decide whether to issue a passport to the child;
3. a final decision of the family court in which the court agrees to issue a passport to the child without the consent of the other parent;
4. a complete copy of the Polish birth certificate, which shows that the father of the child is unknown (there was no acknowledgment of paternity or court determination of paternity),
5. a copy of the other parent's death certificate.

A child under 5 years of age does not have to be present when submitting the application.

IMPORTANT! Parents no longer fill out the application by hand.

You can submit an application for a passport or a temporary passport for a child by the consul of the Republic of Poland in electronic form during the passport visit. The consular employee will complete the application using an electronic form, and the parents will confirm the correctness of the child's data and consent to the issuance of the passport by signing on the device designated for this purpose (Signature Pad).

• Color biometric photo of the child - read what the photo should look like.
Attaching a photograph to the application is mandatory. It is not possible to exchange or send a photo later. If the photograph is not attached to the application or does not meet the requirements, the passport authority shall inform the applicant that the application has not been accepted.

• Valid passports or valid parents' ID cards.

• Child's valid passport or valid ID card, if issued.

• Abridged or full copy of the Polish birth certificate, if the child does not have a PESEL number.

In the case of an application submitted by a legal guardian or guardian, documents issued by the court confirming the right to submit the application on behalf of the child must be presented.

NOTE: in case of justified doubts as to the identity or citizenship of the child or other data needed to submit an application for a passport document, the consul may ask for additional necessary documents.

• The fee for issuing a passport for a child under 5 years old is EUR 35.00.
• Fees are accepted at the cash desk of the consular office at the time of submitting the application.
• The fee is charged [please list the forms of payment available at a given consular office, eg in cash, by payment card].
 
You will not pay less if you apply for a new passport for your child before the expiry of the current passport - if:
• the child's details in the passport have changed, for example, his/her first name(s) or surname (the passport must be replaced)
or appearance in a way that makes it difficult to identify the child,
• there is no more space in the child's passport for further visas or stamps confirming the crossing of the border.

When you apply for a passport, a consular officer will give you an estimated collection date. You will also receive a written confirmation of application receipt. On the receipt you will find the child's passport application number, which is useful if you want to track the progress online.

See if your child's passport is ready. 

If you need the child's ID document urgently, for example because of a coming trip, you should also apply for a temporary passport that will cover the waiting time for the delivery of the biometric passport. You will get it sooner than the biometric one. 

Please note If you are planning to take your child abroad with the temporary passport, check current rules of entry and stay of foreigners in the country of destination. Make sure that the child can cross the border with the temporary passport and check the required validity period (at entry and departure).

The child's passport can be collected by either of the parents (guardians) personally in the same office where the application was submitted.

On the collection day of the new document, bring the child's previous passport (or temporary passport) to the consulate - current passport will be cancelled, except for pages with valid visas, and returned.

Please note, in justified cases, especially if the travel to the consulate is particularly difficult, the consul may depart from the requirement to collect a passport in person. Discuss this point as you submit the application.

Remember that you will have to submit a reasoned written request to the consul to depart from the requirement to collect a passport in person (the form is available at a consular office) and pay the postage. 

The consul will send the passport to the indicated address via postal or messenger service operator, ensuring the security of the document, if possible.

The consul will cancel the current passport or temporary passport before the new one is sent. 

If you know that your child’s passport is ready for collection in the consulate, send your child's passport (or temporary passport) to the consul for cancellation. The consul will return the cancelled document together with the new one.

Remember that the consul will not send passports to citizens staying in Poland.

It is also not possible to send a passport issued by a domestic authority (province governor) to a person staying abroad.

How to obtain the first passport for a child born abroad?

First, you need to register a foreign child birth certificate in Poland (see LINK) to obtain an abridged or unabridged copy of a Polish birth certificate. The Polish birth certificate is necessary to obtain the PESEL number which is then entered in a biometric passport.

How to obtain the PESEL number?

The consul applies for the PESEL number to be issued in connection with a passport application for a child who is not yet a PESEL holder. 
Please remember that the consul does not apply for the PESEL number to be issued under any other circumstances besides the issue of a passport or a temporary passport.

What if one parent cannot apply in person?

The parent who cannot apply in person may consent to the issue of the document by writing a statement of consent. Authenticity of the signature on the statement should be confirmed by a notary or a passport authority (consul, province governor).
