E-mail registration rules:

• Your e-mail message should include data of the persons indicated in the applications, like: name and surname of the foreigner, citizenship, passport number of the foreigner, his/her current residence permit status (visa issued by Polish authorities, Polish residence card, visa issued by another Schengen state, visa-free travelling, illegal stay) as well as the date of the legal stay of the foreigner. In addition, you must provide the names and surnames of the foreigner's family members - for whom applications will be submitted along with their passport numbers and dates of their legal stay. Additionally, provide us with your telephone number and the name and surname of your representative.

• Within 3 working days you will receive an e-mail message regarding the next coming available date at the Masovian Voivodeship Office to submit your application.

• The order of registration is determined by the date of legal stay, which is verified by the clerks while submitting the application.

• You need to provide us with reliable personal data - consistent with the facts. Incomplete data disable us from booking a visit for you. Our Foreigners Service Point reserves the right to refuse to accept applications from foreigners who had provided false data during registration. There is no possibility to change the name of the registered person.

• Foreigners registered during an online booking system should arrive at least 10 minutes before their planned appointment and approach their indicated line. Coming late - more than 5 minutes may result in refusal to accept documents by the Clerks.

• Persons arriving in the Foreigners’ Service Point - for their scheduled booked appointments do not have to collect any additional numbers from the ticket machines, a printout of an e-mail is a confirmation of the appointment

By booking an appointment at the Mazowiecki Voivodeship Office, Department for Foreigners, in regard to submitting an application for stay legalization, you declare that while providing your name and surname, date of birth, telephone number and a citizenship, you agree to the processing of your personal data by the Mazowiecki Voivode in order to implement the online booking process of appointments based on art. 6 sec. 1 point a) of (EU) Regulation no 2016/679 of the European Parliament and of the (EU) Council as of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free flow of such data, as well as repealing the Directive no 95/46 / EC (The EU Official Journal L 119 as of May 4, 2016).

During the appointment booking process, the following data should be provided: name, surname, date of birth, citizenship, number of the currently held residence permit, its expiry date, name and surname of the representative and contact telephone number.

1. The above-mentioned data is obligatory for the purpose of verification on the day of submitting documents to the Foreigners Service Point and obtaining your ticket number.

2. Temporary residence permit applications are accepted in Warsaw at

• 3/5 Bankowy Square (entrance: G)

• 28 Jerozolimskie Avenue

3. All applications regarding permanent and long-term EU residence are accepted only at Foreigners Service Point at 5/11 Krucza str..

4. A person registered through online booking system is obliged to arrive in the Foreigners Service Point – to a proper line - at least 10 minutes before his planned appointemnt.

5. A delay of more than 5 minutes may result in refusal to accept documents at the Foreigners Service Point.

6. The foreigner who registered his name and booked his appointment must arrive personally. In other case it will result in the refusal of application acceptance. The only exception are minors under 6 years of age.

7. Foreigners should attend their booked appointment with a complete set of required documents. All needed information in this regard can be found on the website of Mazowiecki Voivodship Office: https://www.gov.pl/web/uw-mazowiecki/sprawy-cudzoziemców.

8. Personal data of the foreigner who booked his online appointment cannot be changed.

9. In case of more than one appointment booking made for the same indicated personal data - the administrator reserves the right to delete the second and subsequent entries.

10. In case of providing the incomplete data, false data, or booking a wrong queue or Foreigners Service Point location it will result in the cancellation of the booking or an absolute refusal to accept the submitted application.

11. Providing residence address other than the area of the Mazowiecki Voivodeship results in the refusal to accept the application at the Mazowiecki Voivodeship Office.

12. Entering into the appointment booking form some offensive, ridiculous or violating social coexistence rules, results in the cancellation of the booking and so blocking of the user's account.
